IT Asset Order Form

This form is used to order (IT assets) for any new hire or current employee of a PPC Operating Company.

The person requesting the IT asset(s) needs to complete this form to begin the process. PPC Information Technology will not process any orders without a form being completed.

Once your order form is submitted, a ticket will be generated and assigned to begin the fulfillment process. You will be kept abreast of the status of your order via the ticket.

While we try to stock frequently ordered items, supply chain or vendor issues may sometimes impact the delivery and the completion of orders.

We will do our best to communicate these delays and recommend planning accordingly when ordering.

Please allow 5 -7 business days from the time you place your order to the fulfillment of your order.
